Medical Error in hospitals

Phlebotomy error outside hospitals
   * Basic sanitary precautions are ignored
   * Rudimentary medical precautions ignored
   * Equipment is often outdated, incompatible, or contaminated

Error extends to evidentiary blood draws
   * The vast majority of phlebotomists have only 6 weeks of total training; some less - they don't recognize problems.
   * Many draws done in hospitals aren't even performed by hospital personnel
